Virtual Behavioral Health Services with aptihealth

Glens Falls Hospital is proud to partner with aptihealth to provide access to virtual behavioral health services. All patients from any our seven primary care practices can now access integrated physical and behavioral care through the aptihealth platform within days. aptihealth’s supportive care team model and patient-driven care program has been shown to decrease symptom severity by over 50%, resulting in improved health outcomes.

Following an initial screening with one of our providers, patients in need of behavioral healthcare are referred to aptihealth and complete a comprehensive online 15 life domain self-assessment. Patients are then matched with a behavioral health therapist who specializes in their areas of need, and together they develop a personalized, goal-oriented 90-day care plan that includes convenient teletherapy, medication management as needed, and unlimited messaging between therapy sessions. The aptihealth platform enables progress to be tracked weekly and the dedicated care team, including the patient’s primary care doctor, therapist, prescriber, and care manager, remain connected to support your care and path to wellness. Quality Care at Convenient Hours

In order to accommodate your busy lifestyle, most of Glens Falls Hospital’s primary care providers offer early morning/late evening or weekend hours.

If you are in need of clinical advice after your primary care office has closed, please call the office and you will be transferred to an operator who will contact a provider to return your call promptly.

We also offer urgent care services for non-life threatening conditions at the Urgent Care at Cambridge Medical Center. In addition, the Emergency Department at Glens Falls Hospital is open 24/7. If you are having a medical emergency, please call 911.

Board Certified in Oncology, Hematology and Internal Medicine

Dr. John Stoutenburg joined the C.R. Wood Cancer Center in 2005, becoming Associate Medical Director in 2013 and Medical Director in 2015. He currently serves as the Chair of the Cancer Committee and is an active member of the Medical Group Steering Committee and Pharmacy & Therapeutics Committee. Dr. Stoutenburg is the Physician Champion for clinical research for genitourinary and gynecological cancers.

Dr. Stoutenburg received his Bachelor of Science in Biology from SUNY Binghamton, and went on to SUNY Upstate for Medical School. Dr. Stoutenburg completed his internship and residency in Internal Medicine at the University of North Carolina at Chapel Hill, followed by a fellowship in Hematology and Medical Oncology at Columbia University Medical Center in New York.
